VOCAT Counselling Paperwork Western-VAP

Thank you for agreeing to provide counselling services for the above-named client/s. The Western Victims Assistance Program (VAP) will pay for sessions of counselling at per session for an individual consultation.

Please note the following very important information:

• Victims Assistance Program is limited to funding 3 counselling sessions per client.
• We are unable to pay for missed sessions.
• The client is required to sign for each session that they attend with you.
• Please email us if the client we have referred to you fails to respond to your communications (within two weeks after your initial contact). We will then follow up, where appropriate, with the client.

Upon completion of all of the required sessions, please send your invoice made out to including the attached form, to the following address for payment. Please ensure invoice contains ABN, invoice number and banking details for direct payment.

If you are billing each session as you go, please send a copy of the form with the client’s signature with each invoice.
